How it Works

  1. Please fill out the relevant form below to request funding for equipment or to cover registration fees and related expenses.
  2. All requests will be assessed by our Executive Director and YAF vendor partners as needed.
  3. If you applied for registration fees, we will work with the program to pay the fees on the child's behalf.
  4. If you have applied for assistance with equipment, you will be issued with a special gift certificate to cover the cost of the applicable sports gear.
  5. Equipment gift certificates can only be redeemed at the relevant YAF vendor store. Each gift certificate has a unique QR code that will be scanned during redemption. You can print the certificate or show it on your phone.
